We are accepting prekindergarten and kindergarten registrations for the 2013-2014 school year. Children are required to be five years old on or before December 1 of 2013. Children with birth dates between November 2 - December 1 need to complete a waiver request. Registration forms include a place for parents to indicate their placement preference: - Full day/every day kindergarten (all buildings)
- Full day/alternate day prekindergarten (Alward, Bauer, and Forest Grove)
~ Georgetown and Park students will be bussed to Bauer.
~ Jamestown and South students will be bussed to Forest Grove.
All kindergarten placement requests will be honored. Prekindergarten placement requests will be honored based on section availability. If sections fill up, preference will be given to students with birthdays between September 1 - December 1. Parents will be notified of placement decision by May 31, 2013. Information about future Kindergarten Orientation meetings will be included in the notification letter. Kindergarten Screening Information Parents who are uncertain about placement and desire additional information to help guide the decision, can request a screening by contacting their building secretary. Screening will be held in each of the elementary buildings in March. The teacher will provide a summary of the child’s readiness skills following screening. Kindergarten Parent Meeting Our kindergarten round-up meetings for parents were held on February 26. If you were unable to attend one of these meetings you can view the presentation by clicking here . For building specific information, or a folder of information that was distributed at the meetings, please contact the building principal or secretary. Enrollment Forms Please fill out the forms below and return the completed information to any elementary school. - Complete PK/K Registration Form * This form is a fill-in form. Please complete and print. It does not submit electronically.
- Provide a birth certificate (must be certified with a raised seal). We will make a copy of it for our records.
- Complete Health Appraisal Form which includes dates of immunizations and hearing and vision screening. This form is must be completed no later than August 23, 2013.
- Complete the Internet Use Authorization Form *Please read Student Acceptable Use Policy prior to signing.
- Completed Waiver Request for students with birth dates between November 2 - December 1.
